Henrik Ibsen’s in one of his most revolutionary plays, A Doll’s House, filled his set and narrative with symbols that emphasised the idea that above everything, one must be an individual. Doors, macaroons and the tarantella are all symbols that are used by Ibsen to convey to the audience that the life of Nora and Torvald isn’t what it seems to the naked eye. The doors in the ‘doll’s house’ set, are emphasised, to symbolise the separate ‘world’s ‘ Nora and Torvald live in; the illusive macaroons symbolise the control Torvald has over his wife and the wild tarantella dance is symbolic of Nora’s desire to escape from her restricted and heavily defined existence. Doors in A Doll’s House are not just a wooden blocks that can be used as a thoroughfare between rooms; they are used to distinguish between the two different spheres, Nora’s sphere and Torvald’s sphere. All throughout the play, Nora never enters her husband’s ‘world’. Guests for Torvald were instructed to ‘not come in here (Nora’s living room)’, and they went on into Torvald’s study. As Krogstad ‘slammed’ the door on the way out of his house, Nora’s world was smashed into a million shards of tiny fragile pieces. The character of Nora is sent into a spiral of depression, anxiety and out-right craziness that turned a seemingly normal dance into a 19th century movement of oppressed emotions. The macaroons that Nora possesses in the beginning of the play are more than just a common snack. Nora has the macaroons in the early stages of the play, with Torvald around. They give the audience the knowledge of Nora’s child-like behaviour and emotions. All through ‘A Doll’s House’, Torvald treats Nora like an ‘inexperienced child’, and the macaroons are one of many indications of this. She offers it to guests as they enter her ‘world’, but not to her husband, in which she hides them from him. The child-like behaviour is also witnessed in the scene where Nora is playing with her children, calling them ‘little dolls’ and playing with them like they are her friends and she’s not their mother. This child-like behaviour was encouraged by Torvald, by simple nicknames, such as ‘skylark’ and ‘mockingbird’, to make Nora be more immature towards Torvald and keeping her youthfulness that he li ked so much. The infamous dance, the tarantella, was an expression of the oppressed society that woman had to go through in the 19th century. The tarantella was used to convey emotions that woman couldn’t express in normal, everyday life. They used raw energy to express that raw emotion. In ‘A Doll’s House’, Nora uses, just as other woman have, to express and release the oppressed emotion that was given by Torvald towards her. In the scene where Nora is dancing for Torvald as practice of the dance and Dr. Rank comes in, Torvald keeps telling Nora to ‘stop it’ and ‘do it as I said’. Nora is completely oblivious to Torvald and keeps on dancing in her own style. Dr. Rank comes in and takes over Torvalds role as piano player to fuel the flame that Nora was burning. The dance becomes more and more wild as the scene progresses, until the music stops and Nora is just ‘still’. The oppressive moment of woman in the 19th century created this dance. ‘A Doll’s House’ uses this symbols of child-like behaviour, different worlds and an oppressed dance to show the audience that the world of the 19th century wasn’t a blissful place where men and woman were equal in everything they had rights to, like they do now. Torvald showed that he was a man of conformity and tried his best to bend his family into the social norms of the time. Torvald never let Nora have her way and treated her like a child, making her regress into one and even making her be his child. She was scared to wrong by her husband, even hiding a simple sweet from him so he wouldn’t find out that she was eating it. By the end of ‘A Doll’s House’ , Nora grows up. After the problems arisen by Krogstad, she learns that her husband isn’t her saviour anymore and she doesn’t love him. The party ended when Torvald wanted to leave. Nora got home and changed out of her formal attire, and into outdoor wear, making Torvald wonder, ‘Why are you dressed like that?’ The taking off of her formal clothes gives the impression that Nora is now taking off her old life, her oppressed life, and starting afresh. After an emotional discussion with Torvald, she ends up leaving him and the children. Walking out of her house, through the door, again is symbolising the end of something. This time it’s Nora’s life with Torvald. As the door closes, this time it’s Torvald who breaks down, mirroring Nora’s emotions when Krogstad leaves the  letter in the letter box. Symbolisation is a major part in any play, even more so in ‘A Doll’s House’. The emotion scenes with Nora, Torvald, Dr. Rank and even Krogstad wouldn’t be the same without objects such as the doors or macaroons, or the two different spheres dividing Nora’s world and Torvalds. The dance itself plays a vital role of showing emotion without the notice of Nora’s own husband. Polycystic Ovary Syndrome

Introduction: Polycystic ovary syndrome (PCOS) is a common endocrinology disorder affecting 15–20% of women in reproductive age worldwide (1).   PCOS characterized by hyperandrogenism and high level of insulin resistance which leads to the dysfunction of the hypothalamic-pituitary-ovary axis, resulting in anovulation and menstrual irregularity (2-6). PCOS is considered the most common cause of anovulatory infertility. Around 90%–95% of an-ovulatory women visiting infertility clinics have PCOS (7). The current first-line infertility treatment in women with polycystic ovary syndrome is Clomiphene Citrate. However, it has drawbacks, including its overall poor efficacy (only a 22% rate of live birth with up to six cycles of clomiphene (8)), ovulation rate of only 46% (9) and an undesirable side-effect profile, including mood changes and hot flushes. Failure either to ovulate (clomiphene resistance), or to conceive with ovulation (clomiphene failure) often leads to the use of more expensive treatment options for infertility that associated with higher multiple pregnancy rates and an increased risk of the ovarian hyperstimulation syndrome (10). Insulin-sensitizing agents, such as metformin, have been used successfully to treat women with PCOS (11). However, metformin also has multiple undesirable gastrointestinal side effects such as nausea (61%), vomiting (30%), and diarrhea (65%). (12,13) Furthermore, many women who have PCOS are not satisfied with pharmaceutical treatment. In a survey done on women with PCOS, 99% expressed their desire for effective treatment alternatives to fertility drugs, (14) and as many as 70% of women with PCOS use complementary medicines (15-16). Herbal Medicine is still widely used from world population for approximately 75-80%; this is because that herbal medicine is considered more culturally acceptable, less dangerous and more natural form of treatment (17). In Saudi Arabia, approximately 80% of the population use herbal medicine, and one of the main reasons is the ease of use of herbs that could be taken orally without painful procedure or invasive methods (18). Cinnamon, a commonly used spice, has become a natural product of interest because it has been considered to have health benefits, such as reducing blood glucose, total cholesterol, LDL cholesterol and decreasing insulin resistance (19). Cinnamon increases insulin sensitivity likely because of its effect on intermediate metabolites acting at the cellular level (20). Several studies stated that polyphenol polymers isolated from cinnamon could increase insulin-dependent glucose metabolism. (21-23) Cinnamon usually causes no serious side effects. The reported adverse events of cinnamon include headache, heartburn and menstrual cramps (24). In general, herbal medicines are better tolerated than pharmaceutical medications as shown in a systematic review for critical evaluation of clinical efficacy and adverse events of herbal remedies (25). Cinnamon extract may offer a low cost, readily available and relatively easily implemented means of reducing insulin resistance and thereby improving ovulation in women with PCOS.2. Literature Review: A randomized, double-blinded, controlled trial investigating the effect of cinnamon on menstrual cyclicity in women with PCOS, women receiving daily 1.5 g of cinnamon treatment showed significant improvements in menstrual cyclicity, whereas patients receiving placebo did not. Sampling from different patients in the study showed the luteal phase serum progesterone (progesterone level >3 ng/mL), sonographic visualization of corpora lutea, and pregnancy which all support that the bleeding has resulted from ovulatory cycles, rather than merely changes in menstrual flow (24). That suggests that cinnamon supplement may have an effect on ovulation induction in women with PCOS. Regarding the effect of cinnamon on insulin resistance, a study conducted in 2017 involving 66 women with PCOS in a randomized, double?blind placebo?controlled clinical trial. The women in the first group treated by cinnamon capsules 1.5 g/day for three months and the second group by placebo capsules. The homeostatic model assessment for insulin resistance (p = .014) reduced after the three months in the cinnamon group compared with the placebo (26). Another randomized control study concluded that oral administration of 1g of cinnamon extract daily for eight weeks was well tolerated and improved insulin sensitivity in nondiabetic women with PCOS (27). A double-blind randomized control study, comparing the effect of cinnamon and metformin on insulin resistance in 112 women with PCOS taking 1g of cinnamon versus 1g of metformin daily, shows that both significantly decreasing the insulin resistance (28). A study in non-PCOS patients with elevated serum glucose, 173 patients were enrolled and given 500mg of cinnamon daily for two months, showed a significant reduction on fasting insulin, glucose, total cholesterol, and LDL cholesterol and enhanced insulin sensitivity (29). There are many studies conducted on metformin as insulin-sensitizing agent on ovulation in PCOS women. In a systematic review, forty-four trials (3992 women) were included for analysis, 38 of them using metformin and involving 3495 women, clinical pregnancy rates were improved for metformin versus placebo (pooled OR 2.31, 95% CI 1.52 to 3.51, 8 trials, 707 women) and for metformin and clomiphene versus clomiphene alone (pooled OR 1.51, 95% CI 1.17 to 1.96, 11 trials, 1208 women) (30). Since the cinnamon have been suggested to improve the insulin resistance in previous studies and taking into account the side effects of metformin, a cinnamon supplement may offer a good and natural alternative to metformin as an insulin-sensitizing agent and thereby improve the ovulation in women with PCOS.3. Rational:The rarely and non-serious reported side effects of cinnamon along with the cost-effectiveness and availability lead to the need of exploring the effectiveness of cinnamon supplement as a complementary medicine for ovulation induction in women with PCOS.4.1 Aim:To determine the effectiveness of cinnamon supplement and clomiphene citrate (CC) combination on ovulation in women with PCOS.4.2 Objectives:Primary Objective:To compare the effectiveness of cinnamon supplement in combination with clomiphene citrate versus clomiphene citrate alone on ovulation in women with PCOS, in King Abdulaziz University Hospital in Jeddah 2018.Secondary Objectives:To measure the difference in insulin resistance after three months of cinnamon supplementation in women with PCOS in King Abdulaziz University Hospital in Jeddah 2018.To determine the effect of cinnamon and CC combination on menstrual cyclicity, approximated by menstrual frequency, during the study period, in women with PCOS in King Abdulaziz University Hospital in Jeddah 2018.To measure the pregnancy rate in women with PCOS in King Abdulaziz University Hospital in Jeddah 2018.To evaluate the quality of life in women with PCOS in King Abdulaziz University Hospital in Jeddah 2018.Materials and Methods Place of study:The study will be carried out in King Abdulaziz University Hospital, Obstetrics and gynecology clinics, in Jeddah 2018. Exclusion criteria:Current pregnancy or lactation.Current use of treatment of infertility.Established diagnosis of diabetes mellitus.Use of Insulin-sensitizing treatment within the past three monthsHormonal treatment involving estrogen or progesterone within the past three months.Known hypersensitivity to cinnamon.Use of statin medication.Any other supplements that contain cinnamon within the past month.BMI 35.Sample Size:To detect a clinically significant difference of 30% between the previously reported systematic review of ovulation rate (~46%) (9) with a one-sided 5% significance level and power of 80%, a sample size of 82 participants (41 per arm) is required. (Fleiss, Statistical Methods for Rates and Proportions, formulas 3.18 &3.19). Randomization:The participants will be randomized in 1:1 fashion using a computer program by trained assistance. Allocation concealment will be ensured using similar bottles labeled by letters A and B to receive either the combination of cinnamon supplement and clomiphene citrate or clomiphene citrate with placebo. The intervention allocation will be blind for both investigators and participants.5.6 Intervention:The Cinnamomum cassia  (C. aromaticum) capsules will be purchase from Spring Valley company (distributed by Wal-Mart Stores, Inc. USA) by the investigator. Placebo capsules will be prepared by Batterjee Pharma Company and will contain 450 mg of starch and 50 mg of cinnamon powder (to improve blindness regarding taste and odor). Shape, size, and color of placebo capsules will be completely similar to the cinnamon capsules.Cinnamon and placebo capsules will be provided to both groups monthly for 12 weeks. Participants will be required to consume two capsules twice per day after meals (2g/day). The last packages of capsules will be checked at the end of the month, and the number of remaining capsules will be counted; after that, new packages will be delivered to patients. All capsules will be given simultaneously with the clomiphene citrate medication. Participants will be asked to keep their normal lifestyle including daily food and physical activity level.The 2g dose of cinnamon is chosen based on published clinical trials in patients with PCOS (8,15,16,17). All participants in both groups will be followed for three months or until pregnancy confirmed. The three month-period has chosen based on a systematic review that showed the effect of cinnamon could be as early as 4 weeks and up to 18 weeks (31).5.7 Adherence:Compliance with diet and medications and interval progress will be monitored with monthly visits with the investigator or his trained assistances along with SMS reminder or WhatsApp messages. Also, participants will be asked to return all study packs (including empty bottles) at every monthly visit.5.8 Patient safety:Patients will be monitored monthly during the study period, and any occurrence of adverse events will be recorded.5.9 Study Outcomes:Primary outcome: Progesterone level > 3ng/mL (ovulation confirmation) or pregnancy confirmed.Secondary outcomes: Changes in insulin resistance, menstrual cyclicity, pregnancy rate and fertility quality of life at the end of the study in comparison with the baseline values. Procedure:The study will be initiated after obtaining approval from the Unit of Biomedical Ethics in King Abdulaziz University and approval from Saudi Food and Drug Authority. All patients will be explained the procedure and risks involved in common, understandable languages and a written informed consent will be obtained. First visit: Patients meeting inclusion and exclusion criteria will be evaluated during the early follicular phase (day 3-7) after a spontaneous or induced menses (medroxyprogesterone acetate 10 mg twice?daily for 5 days) for: Baseline blood work:o (Hormone profile: FSH, LH and testosterone levels).o In day 21 (luteal phase) patients will be tested for progesterone level.o Insulin resistance using homeostasis model of insulin resistance [HOMA-IR] and Quantitative Insulin Sensitivity Check Index [QUICK-I].? QUICK= 1 / (log(fasting insulin  µU/mL) + log(fasting glucose mg/dl))? HOMA-IR= (FPI(mU/l) Ãâ€" FPG (mmol/l))/22.5 Height and weight will be measured and recorded for body mass index (BMI). Diagnosis of oligo-/anovulation will be based on a menstrual pattern of oligo/amenorrhoea (cycle >35 days) and/or a low mid-luteal serum progesterone concentration. The IPAQ form comprises walking, moderate- intensity and vigorous-intensity activity and will be expressed as metabolic equivalents per minute (MET-min) per week. The levels of physical activity will be categorized into low, moderate and high, based on the IPAQ criteria. Statistical Analysis:Statistical analysis will be performed using SPSS 25. Data will be expressed as mean  ± SD for continuous variables and percentage for non-continuous variables. Normality tests will be assessed through Shapiro-Wilk tests carried out on each parameter before analysis. â€Å"A village singer† portrays the internal conflict, the bitterness and responses of Candace when she was dismissed from the choir that served for forty years. The story partially points out the social norm that set down for women. In this community, women are not considered equal and have the same feelings as men which represent through Reverend Pollard and Williams Emmons. Williams Emmons is three years older than Candace, but he still holds his choir leader position. If they complain that her voice has worsened, Williams's voice logically must have the same situation as her. However, Emmons is not dismissed and remain his choirmaster position. The minister just like Candace also serves at the church for forty years. He hesitates of his speech and could not keep the freshness for his sermons. He still can stay in the church and continue his duties since nobody asks him to leave his position and gives him a photograph album. He critics her voice with Alma and supports the dismissal. Even Candace's nephew, Wilson Ford, threats to throw her organ out of the window if she continues to disturb Alma's solo. He does not express any sympathy or even gently discuss her grief. She also feels hurt and betrayed by members of the choir since they celebrate a surprise party for her and leave a photograph album with the letter informing her dismissal from the choir. However, the way that Candace responses and against to conflict is full of anger, foolishness, disregard, and arrogance. She says that the member of the church pretends to be a Christian; however, she also goes against what the church teaches. She uses photograph album as a footstool, disturbs Alma's solo, refuses to pray â€Å"‘I don't see any use prayin' about it,' said she. ‘I don't think the Lord's got much to do with it, anyhow'† and challenge other people to stop her † I'd like to see anybody stop me.† Besides that, the story carries the message of kindness and forgiveness. At the end of the story, Candace forgives to all people who have wronged her and also ask for the forgiveness from those people. She apologizes to the minister, reconciles with Alma, and forgive Wilson.

Four hours seems to be the minimum amount of time one can sleep per night in order to coherently function during the day. A lack of sleep can have grave consequences, greatly caused by the inability of your immune system to work without rest. Sleep is a multistage cyclic process, involving five steps. Fast waves, called "Beta" waves are those which result from being awake. Once you become sleepy and close your eyes, your body relaxes and your brain waves greatly slow down, becoming "alpha" waves. By keeping an open mind and fully taking in what he learned from the different culture of his enemies he is able to find a peace within himself that he has never felt before and a new path of life that he can follow happily with no regrets.The movie does have awesome historical accurate battle scenes but it also has a deeper story about how a different culture that can seem strange at first effect someone in a good way. After a childhood full of cruelties, disappointments, and being raped at the hands of her mother’s boyfriend, Maya Angelou survived and became the most important civil, human, and Women’s Rights Activist. After her rape she was silent for 5 years, and only through the encouragement of her grandmother did she start to write and act. Her writings won her world acclaim and she was nominated for the National Book Award, a Tony Award and a Pulitzer Prize. She is indisputably one of the most influential women in history. Her poem â€Å"I know why the caged bird sings† shows her grace and skill as a poet, and a humanitarian. â€Å"I know why the caged bird sings† is a six stanza poem. The poem has a very traditional structure. The first two stanzas are triplets, followed by a quatrain, then another triplet, and ends with a quatrain. It has a very simple but effective rhyme scheme. The triplet stanza has a AAB rhyme, and the quatrain stanzas have a AAAB rhyme. This poem is lyrical and intense. The themes presented in this poem are of a freedom and â€Å"triumph over adversity (Arensberg 273)†. In stanza one Angelou describes how birds in the wild have unbound freedom. She writes â€Å"leaps on the back of the wind†¦ dips his wings in the orange sunrays, (lines 1-3)† about the free birds activities. Her words affect the senses, and the reader feels the freedom the birds experience in flight. She brings nature and the outdoor elements to life and the audience is right there at play with the birds. Freedom abounds! In stark contrast, the next stanza snaps the reader back from their dreamlike state. There are a series of very strong words that create a feeling of depression, and claustrophobia. Angelou explains the life of a caged bird. â€Å"narrow cage, bars of rage, and wings clipped. (lines 3-6)† These phrases create a sense of dread and imprisonment. This evokes sympathy for the bird, and thoughts about if we were also imprisoned (Arensberg 280). The reader begins to think what is life unfair and, why that bird? and further, why me? â€Å"But a caged bird stands on the grave of dreams, (line 14)† the poem continues and yet the bird still sings. Angelou comments â€Å" The caged bird still sings with a fearful trill of things unknown but longed for still (lines 17-18)† about the bird who displays the only freedom he has at present, his voice. Pierre Walker, in his 1995 article, comments â€Å"that the bird still dreams,â€Å" and he has the determination to overcome and make the best of his situation. There is a single focus for Angelou use of alliteration in this poem. She uses the â€Å"s† sound over and over again – â€Å"his shadow shouts on a nightmare scream† (line 15). The â€Å"s† sound creates the feeling of wind, the whispering of tree leaves, the bird is scared but he sings because he can. He cannot fight his nature. He will find freedom anyway he can, and for that bird it is his singing. The use of the rhyme scheme in the quatrain portion of the poem is AAAB. The inflexibility of the first three lines in stanza 3 â€Å"thrill†¦hill†¦shrill† create the perfect path to the true theme of this poem. When the reader expects another word to rhyme it doesn’t, and the quatrain ends with â€Å"freedom. † The same is true for the last stanza, which is a repeat of the first. Liliane Arensberg explains that the rhyme reminds the reader that â€Å"there is always hope, there is always the will to survive and the promise at the end of the tunnel of light (Arensberg 289)† – of freedom. â€Å"I know why the cage bird sings† by Maya Angelou is a very popular poem. Ofelia takes a great inspiration from fairy tales and while going to her step father she experiences many strange events which link her as being the lost princess of the underworld. Ofelia's step father is assigned to a remote village where he is designated to throw out all the rebels from the territory. Ofelia gets closely related to the servants working with the captain and she comes to know that the servant Mercedes is linked with the rebels but due to her love for Mercedes she does not tell his father about Mercedes. The rebels try to evacuate the captain from their land and in the end of the movie they even get successful in burning the outpost of the captain and finally killing him. Ofelia gets deeply involved in the fairytales and she is asked to pass some tests to enter the world of the underworld where she is supposedly the princess. In the end one of the test leads to her death in which she has to sacrifice her brother. The captain reaches the spot and kills her to get the posession of the child. Politics is greatly involved in Pan's Labyrinth in the form of war and the actions of people. ... This is because of her defiance to live in the captain's world of brutality. The fantasies of Ofelia from one point of view are also political as she is running away from the captain's world and she figures out that she can live in her own world where the captain does not rule. She tries to find her allies in the rebels and Mercedes and thus she sides up with the rebels. The whole movie portrays a political environment in which everyone is fighting for his own cause i.e. the captain tries to defeat the rebels while Mercedes and Ofelia try to end the tyranny of the captain and run away from his world. The film describes the situation of tyranny and dictatorship and emphasizes on the fact that dictatorship can only be ended through political tactics. In the climax of the movie the captain shoots Ofelia while she is giving a test to be called the lost princess of the underworld. The climax of the movie also portrays a way of how Ofelia fought to get herself released from the world of ca ptain. Pan's Labyrinth is a book which shows both the tactics used by the rebels and the fantasies which were seen by Ofelia in the form of politics as they are both done do get away from the shadow of the captain. 'Watchmen' is known to be a great piece of comic work by Alan Moore in 1986-87. Alan Moore is recognized as the best writer in comics after creating such a master piece. His impressive DC series tracks many caparisoned vigilantes, consisting of the inhuman, knowledgeable, crazy, and intelligent but
